Help Me Grow
As a pediatrician, you know that the early years of life are critical for lifelong outcomes, and that intervening early is key. However, it’s impossible to not only know all of the programs available to help families, but also to ensure they connect to the referral you make. Help Me Grow – Long Island can help. HMG-LI is a free resource for families with children aged 0-5 living in Nassau and Suffolk Counties that provides virtual, ongoing care coordination to ensure that families are linked to services that meet a wide range of concerns: anything from basic needs to developmental concerns. HMG specialists can serve families in English, Spanish, and Creole. HMG-LI also offers free access to developmental screens for families and providers, and can grant Maintenance of Certification credit to ABP-accredited pediatricians who improve their developmental screening and referral process. To refer a family to HMG:
- Fax a referral form – we will then keep you updated on what is happening with the family
- OR Tell them to dial 2-1-1, select their language and press 7 for HMG
